2.2.0: style: normalize attribute and phpdoc ordering

Recent collection target changes left a follow-up formatting pass across
fixtures, docblocks, and imports, which would otherwise keep the branch
dirty and make later behavior changes harder to review.

This commit normalizes attribute ordering, import placement, docblock
alignment, and one test closure indentation so the codebase stays
consistent with the repository style conventions.

Side effects: no intended runtime behavior changes; this is limited to
formatting and source ordering cleanup.
