Syncs upstream/main through a2976eb8 with the fork preservation gate completed.

• Updates Claude prompt caching and tool continuations, Codex citations/tool schemas, Gemini document/schema handling, Antigravity signatures/entitlements, and websocket disconnect handling.
• Fixes streaming delivery accounting, truncated-stream errors, and clean EOF completion; adds model detail lookup and configurable catalog sources.
• Integrates auth mutation and reload coordination while keeping immediate cooldown selection, persisted runtime metadata, failure policies, and the existing public SDK interface.
• Preserves Qoder/Qoder CN, CommandCode, Excel models, Codex private instructions, usage monitoring, model-context overrides, single data root, and fork management assets. Excludes upstream promotion, warning-throttle removal, and unnecessary replay batching rewrites.

Explicit payload rules now remain authoritative at final serialization instead of being overwritten by provider defaults.

Validation: full Go tests, protected regression pins, targeted race checks, formatting, and server build passed. Two independent read-only staged audits approved 210 paths (127 retain, 70 combine, 13 exclude). The server acquired and served the fork CPAMC++ management asset from an empty cache with legacy upstream repository migration verified.
