fix black permission screen, API 29 compat, bump to 1.5

- Wrap PermissionRequestScreen in Scaffold to fix black background with edge-to-edge
- Fall back to PhoneStateListener on API 29-30 (TelephonyCallback requires API 31+)
- Bump versionName to 1.5, versionCode to 3

Release 1.4

bump to 1.4 and rework permission flow

Remove loading screen and CHECKING state; permissions are now resolved
synchronously on startup. Permission is requested on demand via a
"Grant Permission" button rather than auto-launched behind a spinner.
