fix(steam): create a valid shortcuts.vdf when no shortcuts exist yet

The fresh-file path was missing the entry index key and the root map
terminator, so Steam ignored the file entirely (worked only when there
were already other shortcuts). Now verified by tests for all three cases:
fresh file, append to existing, and duplicate-name dedup.
