Adds a Code of Conduct to the Boulder project. The content is adapted from version 1.4 of the Contributor Covenant.

After informally consulting with a lawyer we want to emphasize that
while we intend to maintain confidentiality with regard to the reporter
of an incident we can not break any relevant workplace harassment laws.

This commit also instructs contact be made via the community forum.

LGTM, but as discussed we also want to apply this to the Community Forum. I think probably the best way to do that is to put these in place of the https://community.letsencrypt.org/guidelines and link to that from CODE_OF_CONDUCT.md in the Boulder repo.
