Also tweak two placed that are implicated or mention the outdated flows: - getSCTs can be inlined into the RPC method GetSCTs now that it is the only caller. - a unittest for tbsCertificateIsDeterministic mentioned ca.IssueCertificateForPrecertificate but doesn't need to.
