Fixed

• The agents that build a slice no longer stop early, believing their time is up. A slice's time now grows with its tasks, and the agent checks the real time before it stops for it.
• The Plan keeps each slice to at most six tasks, so no single agent's run gets long; larger work becomes several slices that are built side by side.
• After you approve the Plan in the App, Claude Code's QualityLayer band no longer says the agent still asks "Approve the Plan?".
