Create contributor documentation explaining the copyright and license header format required for source files.

Create docs/contributing/copyright-headers.md with:
- Header format examples for PowerShell, Bash, Python, C#, TypeScript, JavaScript
- Placement rules (after shebang, after #Requires statements)
