closed EM recovery, HINT_REFRESH lookups, firewall-gated tokens.

Changelog

Worker mode (FrankenPHP, no kernel reset): a caught unique-constraint violation in the collection dashboard no longer leaves the EntityManager closed for later requests. New BreadcrumbEntityManagerResetter and DashboardEntityManagerSubscriber reset a closed manager through ManagerRegistry::resetManager() on every main request and main-request exception (so public breadcrumb_render() recovers too); CollectionCrudController resets it right after catching the violation.
Worker mode: BreadcrumbCollectionRepository::findOneByCodeAndContextKey() and BreadcrumbItemRepository::findAllForCollection() use Query::HINT_REFRESH, so edits made by another worker or process are visible even when the identity map is not cleared between requests.
Worker mode: DashboardAccessSubscriber and BreadcrumbInlineEditResolver ignore the security token when no firewall handled the main request (no _firewall_context), so a token left in TokenStorage by a previous request is not trusted.
