What issue are you seeing?

I’m consistently seeing Codex Cloud fail at the PR creation step.

Details:

• Occurs across multiple repositories (not repo-specific)
• Tasks complete successfully and generate valid patches
• PR creation fails every time
• Applying the patch locally via git apply works

Environment:

• Date: May 5, 2026
• Using ChatGPT Pro
• GitHub integration enabled

Error behavior:

• Codex generates changes but fails when creating/updating PR
• Same failure pattern across repos

This appears to be an issue with Codex Cloud PR integration rather than repo configuration.

Please advise if this is a known issue or if there’s a workaround beyond manual patch application.

What steps can reproduce the bug?

After Codex writes the update the PR fails to create.

What is the expected behavior?

The PR should be created.

Same issue, for hours I can't open a PR, always getting an error. Weirdly enough, chatgpt can update my repo just fine, while Codex apparently can't write to it, although I tried every possible solution, from cleaning cache, reconnecting, etc..

Ok, I was able to fix it (or at least I hope that helped) - I went and completely revoked everything related to ChatGPT under Accessibility > Applications (both Installed and Authorized) and I was finally able to open a PR.

Prior to that, it would constantly create an empty btanch with no changes upon clicking open PR, so frustrating, as it took me hours.

Hopefully this helps someone.
