Summary

Mentioning @codex review on a GitHub PR created multiple duplicate Codex
cloud tasks. Pressing Stop briefly shows a loading state, but the tasks
return to “Working on your task.”

Some task detail pages temporarily showed “Cancelled,” then later reverted
to “Working on your task.”

Incorrect task repository association

The tasks display:

trillestyoungin/model-router

even though the mention originated from:

anisahaya/IntelliRouter

Environment

  • Codex desktop app version: Version 26.721.81911
  • macOS version: Tahoe 26.5.2
  • ChatGPT plan: Plus
  • Date/time: July 29, 2026, approximately 11:30 PM EDT

Additional information

Repository access was removed afterward, but existing cloud tasks remained
active/stuck.

What steps can reproduce the bug?

Reproduction steps

  1. Comment @codex review on a pull request.
  2. Observe the eyes reaction.
  3. Open Codex cloud tasks.
  4. Observe multiple duplicate “GitHub Mention” tasks.
  5. Press Stop.
  6. Wait for the loading indicator.
  7. Observe the task returning to “Working on your task.”

What is the expected behavior?

Expected behavior

One GitHub mention should create one review task, and pressing Stop should
permanently transition the task to Cancelled.

Actual behavior

Multiple duplicate tasks were created and cannot be cancelled.
