What issue are you seeing?

After upgrading Codex CLI to 0.147.0, every prompt fails before inference when using Azure OpenAI Responses API:
Invalid 'input[0].tools[0].description': empty string.
Expected a string with minimum length 1, but got an empty string instead.
Downgrading to 0.146.1 resolves the issue.
