Fixes

• Stop under-reporting staking payouts after a re-nomination: /accounts/{id}/staking-payouts
  discovered which validators to query from the account's current nominations, which are not
  necessarily the ones it was exposed to in the era being asked about. A bulk scan existed for that
  case but only ran when the targeted lookup returned nothing, so a partial result was treated as
  complete and every validator the account had since dropped was missing, silently and always
  downwards. Nominations.submitted_in now decides: strictly before the era means the current
  targets were already in place and the fast path is correct, anything else falls through to the
  scan. Eras at or after the last nomination are unaffected. (#423)

Other

• Log transaction submissions: POST /transaction emitted nothing at any level, so a
  transaction that went in and disappeared left no record it had arrived. It now logs the extrinsic
  hash and length on the way out and the outcome with elapsed time, both at info, so a send with
  no matching outcome for the same hash identifies a stuck submission. The full payload is logged at
  trace only. README.md records that SAS_LOG_LEVEL accepts full EnvFilter directives, which
  is how the RPC layer is turned up. (#425)
