Remove -std=c++11 from setup.py, as clang doesn't like it when compiling C files.

Add a SQLite VFS which refuses to delete a specified file. This will be used to test the hypothesis that failure to delete the WAL file after a checkpoint causes the next connection to enter recovery and replay the entire WAL.
