Separate onChildRegister from adding child

There was an issue where the registration of a child and the child being added to the list of children was coupled, and causing extra children to be added. This release separates that logic, ensuring only children that do not currently exist are added.

Fix issue with child registration

On child tabs that were long to load, the onChildRegister was never being fired. Now, if a child has never checked in and is checking in, it will always call onRegister.

Activate tree-shaking

Even though there was a tree-shaking es build, it was not being surfaced in the package.json as the module property. This release sets that property so that build tools can consume the version of the package best suited for tree-shaking.

Fix improper publish

Naturally, the network cut out mid-publish on the very first attempt, this version should have all the things.
