Schema-qualify hypopg_reset() and hypopg_get_indexdef() calls so index_advisor works correctly when hypopg is installed in a non-default schema (#14).
Also fixes a greedy comment-stripping regex and narrows deallocate-all to only the index_advisor-owned prepared statement (#17).
