ScreenFocus now stays out of the way when a second display is not connected.

  • Adds a default-on “Pause when only one display is connected” setting
  • Automatically resumes when a second display reconnects
  • Shows Active or Disabled with a short reason in the tray
  • Moves Pause/Resume into the status row
  • Hides irrelevant Accessibility prompts while automatically disabled
  • Refreshes the README menu-bar screenshot for the new layout

Verified with 23 passing tests on macOS.
