Internos 2.0.0-beta.3

Fixes from beta-2 testing:

• Command mode now actually sees your selection. macOS 26 rejects the systemwide accessibility focus query even for trusted apps; selection capture and direct insertion now go through the target app's own accessibility element. Side benefit: dictation had been silently falling back to the clipboard swap every time — direct (clipboard-free) insertion now works as designed.
• Smart cleanup can no longer invent text. An utterance mixing dictation with a snippet or spoken command now skips cleanup entirely (the model was completing the dangling fragments — that's where the fabricated markdown link came from), and any model output containing a URL or link you didn't dictate is rejected outright.

Install: download the DMG, drag Internos to Applications (replace the existing copy).
