refactor(eve): remove channel add surfaces

Summary

Removes the legacy /channels and eve channel add setup surfaces now that /add and eve add provide one registry-driven installation flow.

Before, channel setup had a dedicated multi-channel onboarding pipeline, including channel selection, target resolution, preflight, scaffolding, deployment, and next-step UI. After this change, channels are installed one registry item at a time through the common integration setup path, eliminating the parallel onboarding implementation and its special-case TUI state.

Documentation, command references, status text, and tests are updated to direct users to /add.
