docs: add the official loopy-loop logo to the README#61

Adds docs/images/logo.png as the official repository logo (centered at the top of the README, matching the team-harness layout), and fixes the stale "one or more workers" phrase to match the deliberate single-worker model (decisions.md D2). The SKILL.md rewrite remains tracked separately (improvement-proposals P1.3).
