installer: install shortcuts reliably and remove a legacy per-user copy

shortcuts are now advertised entries, so a per-machine install actually creates the Start-menu and desktop links; the previous per-user-keypath components were per-user data in a per-machine component and silently skipped. a custom action also runs the old per-user (Inno) uninstaller during install, so a leftover copy can't shadow the new one via PATH order or stale shortcuts.

clear held modifiers and the hover-link on focus loss

losing focus with ctrl held (e.g. ctrl-clicking a path, which opens it and switches focus) left ctrl stuck on internally, so afterwards hovering any path drew the ctrl-click link underline with no ctrl pressed. reset the modifier state and any hovered link when the window loses focus, matching how the ime and mouse-button state are already released.
