Fixed: the cover picker no longer leaves downloaded cover images piling up in the settings folder; they're cleared when you close the book popup (#267)
Fixed: the cover grid no longer reshuffles when you apply a cover
Fixed: reverting a book to its embedded cover now updates the display instead of keeping the old cover
Fixed: opening a book with the cover opening effect turned off now shows KOReader's standard "Opening…" message again
