Turn a failed process, rejected report, or quality miss into a durable improvement in workflow or guidance.
- incident summary
- what happened
- what evidence existed
- what gate failed
- what change would have prevented repetition
Usually chief-of-staff, the relevant analysis SME, and security-qa.
- reconstruct the event
- identify the failing assumption or gate
- define preventive controls
- update workflow, prompt, or template
- capture the lesson learned
- distinguish process failure from target behavior
- tie the lesson to a real preventive change
- QA review if the lesson changes external reporting behavior
- lessons-learned note
- workflow or template changes
- residual risk and follow-up actions
Not every rejected report implies a product vulnerability; some incidents are process failures.
