Create Indicator Page

Under the "Create Indicator" header this form will ask for three things: the indicator's title, a description of the indicator, and to select the applicable courses associated with the indicator being created. a list of courses will be displayed hold "Ctrl" for windows and "Command" for Mac to select more than one course. Clicking the "Create" button will create the indicator and add it to the outcome that the add indicator button was selected under. Clicking "Cancel" will cancel the creation of the indicator from this form no information will be saved.
