Create Outcomes Page

This page will allow only admins to create an Outcome. This form only asks for two things the title of the outcome and a brief description in the respective input boxes. The "Create Outcomes" button should save the information given and it will be displayed in the "Outcomes Page"(visit the "Outcome Page" wiki for further information) the "Cancel" button will not save any data and take you out of the "Create Outcomes Page".
