Create Measures Page

This page after the header requests three pieces of information: measure title, associated indicator, and the description for the measure. The description for the indicator will appear after the indicator is chosen. The Button labeled "Create" saves the inputed information and should appear in a card(for information on "Cards" view the "Cards" wiki) the "Measures Page"(for more information view "Measures Page on the wiki") while the button labeled "Cancel" removes the inputted information.
